What is Cardiac Rehabilitation?
Cardiac Rehabilitation is a medically supervised program that includes individualized exercise therapy, lifestyle changes, education, and emotional support for people who have had a heart attack, bypass surgery, or have had other heart problems.

Our Program
Aimed at reducing risk factors for coronary heart disease, our program includes:
An individualized exercise program based on the patient’s medical needs, abilities, and goals. The patient develops strength and endurance through the use of treadmills, stepper machines, stationary/reclining bicycles, arm and leg ergometers, and free weights. The patient’s response to exercise is monitored carefully through continuous heart monitoring and frequent vital signs.
Educational and lifestyle management sessions offered on an ongoing basis in:
- Heart-healthy eating
- Cardiac medications
- Blood pressure management
- Diabetic teaching and nutritional counseling
- Exercising safely
- Stress management
- Smoking cessation
- Weight loss
Our Team
Patient care involves teamwork. Our cardiac rehab team consists of medical professionals as well as the patient’s family members. Our team may include cardiologists, registered nurses, dietitians, pharmacists, physical therapists, exercise specialists, psychologists, spiritual advisors but the most important member in this team is YOU.
THERAPY
Phase 1 (In-Hospital)
Cardiac rehabilitation begins in the hospital following a heart attack (or other acute illness) or 48 to 72 hours post-heart surgery. The goal is to provide education, support, and assistance with walking and/or range of motion. At this stage, you will learn what happened to your heart, what the healing process involves, and what you can expect both during and after recovery. You will also receive helpful information about stress management, a healthy diet, and smoking cessation. Length: A few days (8 sessions).
Phase 2 (Out-patient)
Staff provides continuous heart monitoring, baseline patient assessment, nutritional counseling, risk factor modification, physical activity counseling, psychosocial support, and individualized exercise therapy. The patient is constantly monitored and educated about how to monitor blood pressure, blood sugar, and cholesterol, as well as how to attain and maintain an ideal weight through supervised exercise program with continuous ECG monitoring. Length: Three (3) sessions per week over a 4-to-8-week period (12 and 24 sessions) MAINTENANCE
Phase 3 (Out-patient)
Cardiac rehabilitation involves continued participation (typically three times per week) in a medically supervised exercise program and education for those who have progressed beyond the acute phase of recovery. The program is designed for patients who no longer require continuous ECG monitoring while exercising. The exercise and educational opportunities in the maintenance program reinforce lifestyle changes for necessary for successful recovery. Length: Three (3) sessions per week over a 16-week period (48 sessions)
Phase 4 (Out-patient)
Everything the patient has learned including nutrition and exercise is maintained usually at a cardiac rehab wellness center or in a community health facility or gym. A rehabilitation staff member no longer constantly monitors the patient during this phase but periodic reviews or assessments are recommended. Length: Three (3) sessions per week (144 sessions)

You may discuss the program with your physician and request a referral or we can contact your doctor for a written referral.
